To get started:

1. Create a new card starting with the name you want for your card type and ending in +*tform i.e. example+*tform (This will automaticaly create a card named example as well.
2. Follow the formatting shown in this link to fill out this card and save it.
3. Create a new card and select type 'example' and it should have all the fields you just created.
