Add these to Image's edit mode:
Title text (appears when mouse hovers over image): {{+*title}} [Phrase]
Alternative text (appears when image does not load):
{{+*alt}} [PlainText]
If only one of these is missing or blank, use the other to fill in for it. If both are missing, use the card name.

Could put that formatting in *image+*right+*content and make it a Related subtab for images, but they're often uploaded in a context where that subtab would take a bunch of clicks to get to.
